Danesh Rahimi, M.D.—Medical Directed
Dr. Rahimi is Board-Certified in Internal Medicine and Emergency Medicine. He is also Board-Eligible in Geriatrics. His extensive history and education, coupled with his comprehensive understanding of geriatric medicine, make him the ideal candidate to direct medical care for Millcreek Hospice.
Dr. Rahimi believes that the involvement of both patients and family members is of the utmost importance. He is a strong believer in preventative medicine, palliative care and health education. Dr. Rahimi’s expertise leads Millcreek’s team in specializing in symptom management, end of life care, pain control and treatment in a home environment.
Dr. Rahimi graduated from the University of Utah Medical School in 1985, and completed his residency in Internal Medicine and his fellowship in Geriatrics at the University of Connecticut.
Rev. Cheryl Moore—Chaplain and Spiritual Care Coordinator
The Reverend Cheryl Moore is an ordained Priest in the Episcopal Church. Rev. Moore holds a BS in Social Science from San Francisco State University and a Master’s degree in Divinity from the Episcopal Divinity School in Cambridge, MA. She completed her Clinical Pastoral Education in a geriatric residential mental health facility, and additional post-graduate education in a host of areas.
When she first arrived in Utah in the early 1980s, Rev. Moore was privileged to serve as part of the Pastoral Care Staff at the renowned Holy Cross Hospital. In this position, she was an important member of a spiritual team working in a medical setting to respect the whole person in health and healing. These are values we at Millcreek very much admire and seek to embody—we are pleased beyond words to be working with the Rev. Moore, such an invaluable asset to our team.
The Rev. Moore also served for many years as the Chaplain of Westminister College, where she provided spiritual care, education and support for students, faculty and staff. She worked with both ecumenical and interfaith groups to foster respect for different faiths and spiritual traditions, creating and presenting a much-respected 10-years series of programs on Women’s Spirituality throughout the intermountain area.
Returning to formal employment in the Spring of 2002, after a time home with her daughter, the Rev. Moore re-established an Episcopal campus ministry presence at the University of Utah. She also developed and coordinated educational offerings for the Episcopal Church in Utah, along with special projects for the respected Bishop Irish through May 2005.
The Rev. Moore has been involved in many expressions of ministry over the years by being available as a retreat leader, preacher, educator and consultant for different groups, interests and churches.
As a spiritual leader, Christian, mother, friend and care provider, the Rev. Moore illustrates the standard of care you can expect from Millcreek Home Health and Hospice. Her knowledge of comprehensive and inter-disciplinary care, along with her truly selfless heart, make the Rev. Moore a true guiding light for all of Millcreek’s patients, families, and even our own staff. It is an honor and privilege to have her as a part of our wonderful team.
Lynda Davies R.N.—Clinical Director
Lynda Davies exhibits genuine compassion and is dedicated to the sincere service of health care.
A graduate of Weber State University, Lynda has served in nursing for more than 15 years, in various health care settings. This healthy and extensive experience enables her to deliver the highest standard of care and support to Millcreek’s patients.
Lynda has gained the respect and appreciation of patients, families and fellow team members since she first began working with Millcreek Home Health and Hospice. Her career began at LDS Hospital, and led her to serve as the Director of Nursing for Bonneville Home Health, Superior Home Care, Caregiver Support Network, and now as Clinical Director for Millcreek Home Health and Hospice. Lynda is truly gifted in building bridges between the many segments of the health care industry, including physicians, hospital discharge planners, various agencies and community liaisons. We are thrilled to have Lynda on our staff.
Lori Roberts R.N.—Assistant Director of Nursing
Lori Roberts is a hospice nurse who personifies excellence in geriatric patient care and case management.
With 29 years in geriatric care and service, two years in hospice care, and what seems lifetimes of experience in compassionate interaction and assistance, Lori demonstrates the highest standard of exceptional patient care. Her years of service as a geriatric charge nurse and case manager in numerous long-term care settings have provided her with a practical and fundamental understanding of the needs of home health and hospice patients.
In addition to her direct patient skills and management abilities, Lori also provides in-service modules for our team members. Lori earned her nursing and business degrees as Montana State University. Millcreek Home Health and Hospice is unbelievably pleased to offer Lori’s services to our patients.
Catherine Patillo—LMT, NCTMB
Catherine Patillo is Millcreek’s Licensed Massage therapist.
Cathy brings a added and complimentary dimension to the already-impressive range of treatments available to Millcreek Hospice patients. She is trained in a diverse and clinical variety of holistic medias.
With Cathy on board, we are excited to bring the benefits of integrated holistic influences to our already sound base of scientific and medical knowledge. Millcreek Hospice is certain that by offering diverse and comforting dimensions to our palliative care services, each patient’s journey can be ever more peaceful and serene.
We embrace Cathy as part of our team, and believe her expertise to be an asset to us all.
